Establishment has added storage trailer out of rear exterior door with additional dry storage and three reach in freezers inside the trailer.
Dead roaches on premises. Observed one dead roach in exterior storage trailer. Operator swept and discarded.
Ice buildup in reach-in freezer and/or walk-in freezer. Build up of ice on top portion or reach in freezer and reach in freezer door in freezer 2 in exterior storage shed.
Old labels stuck to food containers after cleaning. On warewashing area storage shelf, clean containers on shelf with old label remnants attached.
Single-service articles improperly stored. In exterior storage trailer, single service containers in box on the floor. Operator relocated.

Duct tape used to repair nonfood-contact surface. Duct taped used to repair reach in cooler door handle.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Reach in freezer in trailer has mold like substance on the door gasket.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer solution in multiple buckets at 0ppm. Operator has fresh sanitizer in three compartment sink to reset.
