Operator must obtain a public food service license from the Division of Hotels and Restaurants within 60 days. Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. Waste water tank does not meet specifications per plan review submitted and approved: 1) The TECK unit is limited to operating on premises of establishment under License # SEA2800449 during renovations for up to 120 days from licensure. Unit must maintain access to potable water and waste water utility connections of the licensed food service establishment and must remain on premises for the duration of this license and any future extensions as necessary. Unit must operate within 300 feet from the licensed establishment under License SEA3916003. Observed waste water tank on pavers on the ground and attached to unit from outside. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days.
