Observed butter packets (68F - Cold Holding) at the server station. The operator discarded the butter packets. Observed cooked chicken (52F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler at the entrance to the cook line. The operator voluntarily discarded the cooked chicken. Observed cooked rice (51F - Cold Holding); sliced cheese (54F - Cold Holding); cut lettuce (50F - Cold Holding) in the top portion of the cold holding unit on the front line. The operator stated the foods had been held for approximately 2.5 hours. The operator placed the potentially hazardous foods on ice packs.

Clean equipment/dishware/utensils stored below soap or paper towel dispenser exposing the items to drips from people's hands. Observed wing tossed hopper machine next to hand washing sink, with wings and seasoning inside and lid off causing possible cross contamination if lid isn't kept on when in use to prevent water/hand soap dripping inside when reaching for hand towels when using sink. Observed General Manager correcting immediately and coaching employee.
