Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee pick up order ticket from the floor and proceed to touch clean equipment and to-go boxes without changing gloves or washing hands. Discussed with employee and operator.
Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Operator fixed and hot water to truck was restored.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Shredded Cheese (50F - Cold Holding); Whipped Cream (51F - Cold Holding) in reach-in cooler to the left of the grill with an ambient air temperature of 50F. Employee moved to freezer and another reach-in cooler after stating they haven been in the reach-in cooler since this morning.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Rice (92F - Hot Holding); Beans (82F - Hot Holding); Chicken (91F - Hot Holding); Beef (96F - Hot Holding); Pork (100F - Hot Holding) in steam table at 2:45 PM. Employee stated items had been in steam table since 12:00 PM and that the steam table was turned off because the food was too hot. Employee started reheating items in microwave.

Mobile food dispensing vehicle service opening larger than necessary for food service. Observed mobile food truck with service window open without screen and door to truck open without screen. Flies are present.
