Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At reach in cooler; cooked rice (50F - Cold Holding). Operator stated not prepared or portioned today, removed from cooler and left out of temperature control for 1 hour, moved to reach in freezer inside commissary.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At steam table; cooked beef (120F - Hot Holding). Operator stated reheated prior to being placed in steam table, held 1 hour, moved to be reheated to 165F.

Mobile food dispensing vehicle disposing liquid waste improperly. Waste water dispensing directing onto ground from mobile food dispensing vehicle. Operator closed valve, wastewater stopped draining.
No paper towels or mechanical hand drying device provided at handwash sink. Operator provided paper towels.
2 Dead roaches on premises. Operator removed, cleaned and sanitized areas.
