Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood filters above cooking equipment.

Single-use gloves not changed as needed after changing tasks. Employee picked up case of food off floor and proceeded to make frozen drinks in kitchen without changing gloves and washing hands. Manager had employee change gloves and wash hands.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Item stored in baskets above cook line fryers: cooked plantains (123F,125F - Hot Holding). Employee stated plantains placed in baskets 15 minutes earlier. Employee reheated and served the plantains.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling vents above make line.

Employee dried hands on clothes/apron/soiled towel after washing. Cook wearing gloves touched waist and apron, then removed gloves and put on gloves without washing hands. Then handled cooking utensils and continued cooking on the grill. Cook wearing gloves picked up lid from floor, then returned to handling cooking utensils and aluminum pan, without removing gloves, washing hands and putting on clean gloves. Discussed with Manager.
Raw animal food stored over/not properly separated from ready-to-eat food. Raw Chicken stored over precooked Chicken in drawers under grill.
Food-contact surface soiled with food debris, mold-like substance or slime. Mold like substance on the underside of the food contact surface of the interior of the ice machine in kitchen. Mixed egg lid fell on the floor and cook placed lid on container.

Employee washed hands with no soap. Employee cracked eggs rinsed hands without soap and put on gloves to handle to go containers. Inspector stopped employee and had employee wash hands and change gloves.
