Commercially packaged Fish thawed in reduced oxygen package labeled to remove fish prior to thawing still in sealed package in reach-in cooler drawers on cook line. Discussed with operator. Operator discussed with employee to place a cut in sealed bag.
Dead roaches on premises. One dead roach observed in corner of bar area.operator cleaned up and discarded dead roach.

Observed Raw chicken (44F - Cold Holding); Raw beef (45F - Cold Holding); Raw Shrimp (45F - Cold Holding) In drawer cooler under grill. Manager stated they just cut this morning at 10:30am. Manager placed ice bath on all TCS foods in drawer. Took second temperature of TCS Foods under 42F and below.
Handwash sink used for purposes other than handwashing. Observed dish stored in hand sink at the end of cook line. Employee removed and stored dish properly.
