Commercially processed reduced oxygen packaged tilapia stored in the walk in cooler bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Operator discarded the tilapia.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. All the following items were stored in the top section of the reach in cooler on the cook line Swiss cheese (54F - Cold Holding); Cut lettuce (57F - Cold Holding); Cut tomatoes (53F - Cold Holding); Sausage (53F - Cold Holding); Cooked potatoes (54F - Cold Holding); Cooked yuca (53F - Cold Holding). Operator stated the items were stored in the reach in cooler for approximately 2 hrs. Employee placed bags of ice on all the items.

Time/temperature control for safety food reheated in a microwave oven for hot holding not reaching 165 degrees Fahrenheit. Observed employee reheating rice in the microwave. Employee removed the rice from the microwave and placed it directly in the hot well without taking a temperature. The reheating temperature of the rice was 139F. Employee was informed and placed the rice in a bowl, then covered it and placed it back in the microwave. Employee removed, uncovered, stirred the rice and took a temperature. Temperature 200F for more then 2 minutes.
