Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee handled raw philly meat and changed gloves without first washing hands. Employee then proceeded to work on preparing multiple food orders.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Threaded hose bibb with attached hose by front hand sink missing vacuum breaker. Manager installed new one.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Bulletin provided to be posted.
