No proof of required state approved employee training provided for any employees. Operator unable to provide proof of employee food safety training for any employees.
Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. The door to the unisex bathroom is not self closing.

Dented/rusted cans present. See stop sale. Damaged/spoiled/recalled food not properly segregated. On shelves in dry storage, not segregated from undamaged cans there is one severely damaged 6 pound 11 ounce can of Nacho Cheese sauce and one severely damaged 8 pound 8 ounce can of caramel fudge sauce. Also on the storage shelves, not segregated from un damaged cans are ten 8 pound 8 ounce cans of caramel fudge sauce that are rusted on the lids and rims.

No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. The operator has moved the Food Service counter and seating from one side of the establishment to the other. Hand sink has also been moved in the front of the store and the ice machine in the back kitchen has been removed and replaced with two upright reach in freezers, there is a new ice machine at the front counter,no plan review for renovations.
Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. There is no hot water at the hand washing sink at the front counter.
