Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed : both chemical dish wash machine indicated 0 ppm for chlorine. The systems were primed and tested correctly.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed : hot holding chicken on the cook line at 69°. This was placed in a frying pan for reheating.
Toxic substance/chemical improperly stored. Observed : spray sanitizer stored on the top ledge of the ice maker. The spray was moved to cleaning.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed : no plan was available for review. One was emailed.

Old labels stuck to food containers after cleaning. Observed : as a food pan came out of the dish wash machine, it still had a date sticker on it.
Stored food not covered. Observed : prime rib on the top shelf of the speed rack in the e walk-in cooler. No cover.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed : at the breader station: fish 46,47°, milk wash 52°, chicken 44°, chicken 45°. All items were ice bagged down. Items were 41° and below at the bottom of the pans. Pans were over stocked in preparation for lunch. After ice bags were added, all items dropped at least 2° in 10 minutes. Ice bags were left in the pans, and the pans were reduced in the amount of food in the pans, the overstock was moved below in an open pan. ALSO, cut leafy greens 48°. on the cookline, drawer, Warning**
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed : the butter on the wait station line, lacking a time stamp.
Manager or person in charge lacking proof of food manager certification. Observed : Mike M, lack of proof oF a certificate. This was found on the ServSafe website. Good until 12-2025.
Old labels stuck to food containers after cleaning. Observed : as clean dishes were coming out of the dish wash machine, date labels were still on them.
Wiping cloth sanitizing solution stored on the floor. Observed lack of Wash cloth solution on the floor at the breader station.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed for the eco lab sanitizer, no test strips or indicator chart.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed a lidded drink on the cook line prep table, this was moved below.

Items are less than 4 hours out of the walk-in cooler. Items are being iced down. ALSO, sour cream and butter, 58-59 on the front line wait station. Fresh ice was in the container below, no water, and not touching the pans.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: multiple cutting boards stained on the cook line. ALSO, the cook line reach-in freezer for desserts has an accumulation of food debris in the bottom.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed: quaternary amonia sanitizer at less then 100 ppm. This was refreshed.
