Food-contact surface soiled with food debris, mold-like substance or slime. Observed ice deflector plate and surrounding area in ice machine soiled. Operator started to clean deflector plate during this inspection.
Floor tiles missing and/or in disrepair. Observed flooring leading to back room chipped/ in disrepair.
