In stand up reach-in freezer in kitchen, open raw beef stored over beef empanadas. Also, open raw chicken stored over open raw beef. Employees rearranged products to proper storage.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Thawed mahi in original reduced oxygen packaging in reach in cooler in kitchen. Packaging states to remove before thawing. Operator slit all packages during inspection.

In-use tongs stored on equipment door handle between uses. Tongs on oven door handle. Operator moved to proper storage.
Plumbing system in disrepair. Hand wash sink behind front counter is leaking and not useable. Another handwash sink operational behind front counter in proximity.
