Handwash sink used for purposes other than handwashing. Observed an employee filling a sanitizer bucket with water from the hand sink on the front line.

Dented/rusted cans present. See stop sale. Observed 10 dented #10 cans of peaches mixed with undented cans on a rack in the prep area. The operator segregated the dented cans.
Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the chlorine solution for the dishmachine at 0ppm due to an empty container of solution. The operator replaced the empty solution. Observed the dishmachine chlorine solution at 100ppm.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed wiping cloth quaternary solution exceeding 500ppm. The operator diluted the solution to 200ppm.

Floor area in the dish area covered with standing water. An employee cleaned up the standing water.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed an in-use metal scoop stored in 100F standing water on the front line. The operator replaced and properly stored the scoop.

Dented/rusted cans present. See stop sale. Observed dented can of corn on shelf. Observed operator removing for return to distributor. Operator explained that they were aware and awaiting return. Educated on segregation of cans until pick up. Operator removed cans and segregated. No stop sale paperwork needed.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed Milk in walk in cooler without a date marked after opening on when it was opened. Educated operator on date marking of dairy items, and observed operator date marking milk.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on reach in coolers with food debris. Observed employee cleaning gaskets.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed inside ice machine, rust like substance and black mold like substance.
Buildup of food debris/soil residue on equipment door handles. Observed equipment handles on reach in's with food debris on inside.
Ceiling/ceiling tile shows damage or is in disrepair. Observed multiple ceiling tiles in back areas of restaurant not properly closed, with gaps, and hanging.
Equipment in poor repair. Observed racks in reach in freezer by cook line with coating peeling off shelving
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup on freezer door. Door frozen shut, ice build up on floor and boxes.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed inside of microwave top area with food debris build up.
Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Observed multiple items in walk in cooler with out labels on them.
Stored food not covered. Observed cut potatoes in reach in freezer on cook line not covered for protection. Observed operator covering potatoes with a lid.
