Live, small flying insects found. Observed approximately 20 flying insects on the ground crawling and flying around far left side of pizza make. Observed ingredients unlidded at pizza make, flying insect observed in pan of minced garlic. See stop sale. Observed approximately 10 flying insects in back kitchen area near dishpit and mop sink area. Person in charge began killing.
Ordered shut on the spot. 7 violations recorded, 4 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Canadian bacon on pizza make stored above fill line at 54F. Person in charge removed excess and placed in unit drawers below.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker missing at mop sink on non chemical side of splitter with black hose attached. Also, observed vacuum breaker on mop sink faucet corroded, missing pieces.
Floor soiled/has accumulation of debris. Floors at front pizza make area with debris accumulated in corners along walls, at base of cabinet at point of sale system with presence of flying insects in area. Also, floor soiled in back kitchen area at dishmachine and triple sink area.
Food stored in a location that is exposed to splash/dust. Observed unlidded container of dough stored below and to the right of the hand wash sink exposed to possible splash from hand wash sink. Person in charge covered and moved below dough press
