Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Upon inspection, 3 compartment sink sanitizer reading 0ppm. Operator emptied sanitizer bag and manually mixed sanitizer solution and achieved reading of 150ppm.

Upon observation, reach-in cooler was not on. Operator turned unit on and added ice to help rapidly cool items. Shredded lettuce 51f after 30 minutes, shredded cheese 50f after 30 minutes.
