Equipment and utensils not properly air-dried - wet nesting. Clean stacked pans on storage shelf across from walk-in-cooler unable to properly air dry. Operator started to seperate pans to properly air dry
Food stored on floor. Observed container of oil stored on floor across from managers office. Employee removed oil to properly store.
No drainboards or equivalent provided for soiled items and/or air drying cleaned items. Observed No drainboard at dishwashing machine for clean items. Operator stated something happened to the previous table that was present.

Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee in back kitchen washed hands for approximately five (5) seconds.
Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee in kitchen touched face/hair, handled sanitizer bucket, and immediately began working with food items without washing hands. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.

Observed Chemical Dishmachine not sanitizing properly, at 0ppm. Manager will discontinue use of dishmachine for sanitizing and set up manual. Manager was able to get Dish machine to work properly. Inspector tested dish machine at 100ppm.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold-like substance around ice chute at drink machine located in dining room. Observed can opener soiled.
No soap provided at handwash sink. Observed no hand soap at hand sink located next to ice machine. Employee provided soap for hand sink.

Ceiling/Vent shows damage or is in disrepair. Observed 2 ceiling air vents across from Dish machine to be missing covers over air vent leaving open hole to ceiling.
