Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee take dirty dishes with gloves on, remove gloves then put another pair of without washing his hands. Addressed and he washed his hand then placed gloves on.

Spray bottle containing toxic substance not labeled. In dish area, blue and yellow liquid bottles had labels wiped off. Operator added new labels.
Bathroom door left open other than during cleaning or maintenance. In back of building, employee bathroom door propped open. Operator closed door.
