Food-contact surface soiled with food debris, mold-like substance or slime. Large can opener was rusted and had encrusted debris on and around the blade.
Equipment in poor repair. The front pizza maketable for pizza had severely torn gaskets on all three lower doors. Educated Operator that keeping these items in good repair is necessary for efficient operation of the unit and to keep these items in surfaces sanitary, smooth and easily cleanable.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Bin containing corn meal had a large scoop stored inside with the handle touching the product. Operator moved scoop to the handle facing up and out of the product.

Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Chicken in containers in the walk-in cooler were dated 1/23 and 2/27. Operator stated staff had failed to change the dates and labels when preparing a new batch.
Handwash sink not accessible for employee use at all times. Bottle of drinking water was observed in the handwash sink in the prep area next to the mop sink.

Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed multiple personal water bottles on food prep surfaces and on top of a container of croutons at the make table.
Food-contact surface not smooth and easily cleanable. Observed three (3) serving plates at the make table severely chipped. Observed chef knife with a missing tip.
In-use tongs stored on equipment door handle between uses. Tongs are being stored on oven handle and stove handle.
No handwashing sign provided at a hand sink used by food employees. Operator printed a sign and placed at sink.
Observed heavy build-up on ceiling of walk-in cooler.
