Observed : in the walk-in cooler, time as a public health control items such as but not limited to: butter packs, honey dew and cantaloupe were returned to the walk-in cooler after morning service. The staff went to discard all time/temperature control for safety items that had been out.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed : lack of time marks for multiple items on the front line. Time marks were being added for the last 3 hours left.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the water spout at the self serve area is not clean.
Handwash sink used for purposes other than handwashing. Observed : the front line hand wash sink at the cook line end had a pot in it. The pot was removed.

Equipment in poor repair. Observed : the gauge for the hot temp dish wash machine did not reach over 172°, when the surface thermometers reached 163°.
Equipment or utensils not designed or constructed in a durable manner. Observed : multiple dish wash machine racks that have broken or are otherwise comprimised.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed : ice sculptures forming in the walk-in freezer, under the fan.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : debris accumulation on top of the dish wash machine.
Worn, torn and/or soiled floors. Observed : the floor in the walk-in freezer has debris.
A return visit. The problems from the previous inspection had been dealt with.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observe the washing machine when a high temp thermometer was put in only reach 143 two times in a row.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in the walk-in cooler: honey dew 54°, cantaloupe 57° and yogurt 55°. These items were on the front line for service, brought to the back, wrapped tightly and rolled into the walk-in cooler on a cart. These items were then vented and rolled into the walk-in freezer, reducing in temperature: Yogurt 52°, cantaloupe 49°, honey dew 47°, in 10 minutes. ALSO, in the front pastry display: butter packs 57°, cream cheese packs 56°. These were placed out for service about 7:00 am. These were moved to the walk-in freezer and dropped to 34° in 13 minutes and were then put in the walk-in cooler. These last two items Corrective Action TakenRepeat ViolationAdmin Complaint**
Toxic substance/chemical improperly stored. Observed quaternary ammonia wipes stored over plastic wrap on the prep table in the kitchen. These were moved below.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the soda nozzles in the self serve area are soiled.
Equipment in poor repair. Observed the temperature gauge for the hi temp dish wash machine does not move during the cycle.
Worn, torn and/or soiled floor. Observed the floor in the storage room by the soda machine s soiled.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sour cream in the kitchen 2 door reach-in cooler, 47°. This had been out for breakfast service, and returned to the kitchen. This was placed in the walk-in freezer and cooled to 41° in 11 minutes.

No soap provided at handwash sink. Observed: the soap dispenser at the 3 compartment sink was not operable. The unit was opened and soap became available. ALSO, on the front line cook area, the soap dispenser would not release soap. The dispenser was opened and soap was then available.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed: cooked shrimp in the hot hold cabinet, 119°. The shrimp was moved to the steamer to bring it to 165° for one second.
Toxic substance/chemical improperly stored. Observed: pre-moistened sanitizer wipes stored next to or over clean dishes or food, on the front line and in the kitchen. Each bagged item of sanitizer wipes was moved to the bottom shelf or rack in that area.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed: by the walk-in freezer door, an ice block has formed between the door jamb and the shelf. A work order has been put in to troubleshoot this situation.

Quaternary ammonium sanitizer in spray bottle/bucket not at proper minimum strength. Observed: a sanitizer bucket on the front line prep corner, less than 100 ppm. This was refreshed to 300 ppm.

Carbon dioxide/helium tanks not adequately secured. Observed: co2 in the room behind the soda machine is not secure.
Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed: sanitizer bucket stored on the front line corner prep counter with bread. This was moved below.
Stored food not covered. Observed: pork at 38F in the walk-in cooler, uncovered. A pan was brought in.
A return visit. The problems from the previous inspection had been dealt with.
