Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine 0 ppm, even after attempts to prime. Three compartment sink set up with 50 ppm reading.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Cook touched pizza after it came out of oven.

The ice machine is not in the same spot as listed it's by a mop sink and reach in freezer by kitchen back door entrance across from employee bathroom and no dish washer is on plans observed a dish washer by three compartment sink. Number 10 equipment is labeled as yogurt stand which is where the pizza slices are sitting at.
