Spray bottle containing toxic substance not labeled. Observed unlabeled spray bottle with chemical inside stored on wire rack next to three compartment sink. Person in charge stated chemical was quatanery ammonium and labeled accordingly.
Carbon dioxide/helium tanks not adequately secured. Observed one unsecured CO2 tank next to three compartment sink. Person in charge secured tank.
Cutting board has cut marks and is no longer cleanable. Observed blue cutting board with grooves and no longer smooth and easily cleanable stored on wire rack next to three compartment sink. Person in charge voluntarily discarded cutting board.
No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign at either of the two hand wash sinks in kitchen. Inspector provided hand wash sign.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled back plates around soda nozzles on soda tower in kitchen.

Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed one opened container of heavy cream in tall reach in cooler across from three compartment sink with no date marking. Person in charge stated heavy cream was opened day prior and labeled accordingly.

Reach-in cooler interior/shelves have accumulation of soil residues. Interior reach in cooler bottom storage area under where sodas are kept is soiled.
Wet mop not stored in a manner to allow the mop to dry. Mop was not hung up to dry. Manager hung mop up.
