Observed chlorine dishwasher machine running at 0 ppm. Operator ran machine and primed sanitizer to 50ppm at time of inspection
Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee handle dirty food equipment then proceed to put on gloves and handle raw beef without washing hands first. Educated operator on proper hand washing for employee to wash hands correctly

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Waitress entered the kitchen, rubbed her nose and started scooping ice for service, without washing her hands fist. Reviewed proper hand washing. She went to wash her hands.
Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Establishment offers ceviche containing raw tilapia, and scallops in ceviche Tulum. Owner stated going forward scallops and tilapia will be served boiled and then marinated.
Operating with an expired Division of Hotels and Restaurants license. Exp. 12.1.24 License was renewed during the inspection.

Steam table: cheese sauce (127F - Reheating) at 11:45 since 10:30 - 125F 12:00 Per operator, cheese sauce is a reheat from 10/2 preparation. Operator returned to stove to reheat to 165+F.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. pork carnitas (110F - Hot Holding) Observed in metal pan on char grill. Per operator, out of temperature for 1 hour. Operator returned to stove, rechecked 165+F.
Toxic substance/chemical improperly stored. Observed Aspirin stored next to seasonings at cook line service window. Operator removed aspirin.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed lack of Vacuum breaker at outside mop sink, splitter added.
