Food contaminated by employees and operator continued to serve food. See stop sale. Observed operator touch food he was cooking with bare hands, taste test it off finger, and then touch the food again for another taste test.
Stop Sale issued due to food not being in a wholesome, sound condition. Salmon in reach-in cooler baring label not removed from reduced oxygen environment before thawing.
Handwash sink used for purposes other than handwashing. Handwash sink at sushi bar has dishes stored in it. Operator removed dishes during inspection.
Water with a temperature of least 85 degrees Fahrenheit shut off at employee handwash sink. Per operator, knob is stripped and needs to be replaced.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Frozen time/temperature control for safety food being slacked at room temperature is no longer frozen solid. Shrimp thawing in ambient air. Operator placed shrimp into reach-in cooler.
