Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chemical dishmachine at 0ppm Cl.
Employee switched from working with raw food to ready-to-eat food without washing hands. Observed cook place raw hamburger on grill and then removed hamburger bun from storage bag and placed on plate and did not wash hands and put on gloves between handling raw meat and Ready-To-Eat hamburger bun. Discussed with employee when to wash their hands after handling raw meat.
Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee prepping food, receive two phone calls on their cell phone, removed their glove, stepped outside to talk then returned to work station and placed original glove back on without washing hands and changing their gloves and resumed prepping food. Discussed with person in charge of kitchen
Nonfood-grade containers used for food storage - direct contact with food. Observed portable yellow container marked Diesel at the end of the bar. Employee advised that the container has margarita mix in the non-food grade container.

Toxic substance/chemical improperly stored. Observed two spray bottles with chemical cleaner stored facing alcohol bottles at the bar.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed "Y" connector at hose Bibb in dishwash area with missing vacuum breaker on the non-chemical side of the "Y" connector with an attached hose.
Establishment not maintaining clam/mussel/oyster tags for 90 days. Observed oyster tags not kept for 90 days and not marked with last date served.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed on the cook line the interior of microwave soiled with splattered food

Observed in the dry goods storage area employee jacket on rack over bag of rice, a hat stored on shelf over the coconut milk, rugs stored with stains crackers, and a backpack on top of the ice machine.
Food stored on floor. Observed box of white vinegar stored on floor in kitchen. Observed four boxes of beef stored on the floor in walk-in-freezer
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on the door and door frame of walk-in-freezer.

Raw or undercooked oysters offered and establishment has no consumer advisory sign provided on wall, menu, placard, table tent or by any other written means. Raw animal foods must be fully cooked prior to service. Observed no oyster consumer advisory posted. Manager posted new oyster signs.
Carbon dioxide/helium tanks not adequately secured. Observed Carbon dioxide tanks not adequately secured in dry goods storage area
Floor tiles missing and/or in disrepair. Observed missing floor tiles in dishwash area
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on exterior of walk-in-freezer door.

Observed mold like substance on the underside of the non food contact surface of the nozzles on the soda dispensing machine at server station. Manager instructed employee to clean.
Old labels stuck to food containers after cleaning. Observed old labels not removed from food containers after cleaning on the clean ware rack.
Plumbing system in disrepair. Observed discharge pipe from sink disconnected from drainage pipe. When dishmachine discharges water, some of the water drains on to the floor through the disconnected pipe.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in a manner not allowed to dry.
