Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker present, placed on side with hose going to chemical box. No vacuum breaker on other side. Explain to operator it ends to be placed on other side of splitter.
Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed strawberries over ready to eat food used for pastries. Operator moved to bottom shelf.

Carbon dioxide/helium tanks not adequately secured. Observed tanks in the bar not secured next to dump sink. Operator secured them.
Establishment charging automatic gratuity without properly disclosing the information on the menu and the bill. Observed Arts and Entertainment fee poster in the kitchen, establishment doesn't have it posted by the host stand and they have an "A&E Fee" on the customer's receipt. Reviewed with chef and front of the house manager that the fee must be disclosed on the menus and spelled out on the receipt.
Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed sanitizer bucket on prep table, operator moved it to the bottom shelf.
