In glass door reach in cooler, ham (70F - Cold Holding); sausage (72F - Cold Holding) per operator during rush places tray on cook line. Placed back in reach in cooler a few minutes prior. Discussed with operator utilizing existing time as public health control procedures for time/temperature control food for safety in tray.

Raw shell eggs held utilizing time as public health control missing product time marking. Operator placed time marker on flats of eggs for 8:05 and 8:40.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the flip top reach in cooler, cooked potatoes (51F - Cold Holding) per operator had out on counter during rush and placed back in 10 minutes before ambient air of reach in cooler 38F. Discussed utilizing existing time as public health control plan to hold cooked potatoes.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink on shelf over clean containers next to the three compartment sink. Employee relocated the beverage.
Equipment in poor repair. Toaster oven on cook line door held shut by wire, door opens in its own if not held closed. Standup reach in cooler has leak of water being caught by metal container. Reach in chest freezer in back storage area has ice build up around the seal.

Spoke to employees during inspection who washed hands.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee place cheese on sandwich with bare hands. Spoke to employee who washed hands and donned gloves.
