Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed their sanitizer bucket at 400ppm quaternary. Operator remade the bucket to 200ppm.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed debris buildup inside their ice machine.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop handles in contact with food in multiple containers. Operator removed some.
