Rodent activity present as evidenced by rodent droppings found. Observed two rodent droppings in unused room at far end of establishment. Person in charge cleaned and discarded.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed 5 boxes of cooked crab with temperatures of 55-56F stored in reach in cooler next to ice machine. Person in charge stated crab was cooked day prior and was not removed from reach in cooler after placement.

Pizza person in charge indicated was for employee stored in reach in cooler near back door.

Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location. Operator did not have proof of having food manager certification. Operator stated they don't work here often, but has one at the other location she manages.
No paper towels or mechanical hand drying device provided at handwash sink. At hand wash station next to dry storage area, employee restroom no paper towels. Operator brought paper towels.
Ceiling tile missing. In showroom where customers can pick out crab, ceiling tile missing. Operator stated they're waiting for part to come in to replace tile.
Ceiling/Walls not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Ceiling tiles in kitchen and dry storage area are not smooth and easily cleanable. In back room next to employee restroom, wall behind large white reach in cooler freezer in disrepair.
Dead roaches on premises. Observed approximately 10 dead roaches. 5 in restroom next to dry storage area. 5 in back of establishment that is only used for storage. Operator discarded.
Employee personal food not properly identified and segregated from food to be served to the public. Employee food not labeled or separate from customer food at reach in cooler where raw shrimp is sold to customers. Employee food removed and segregated.
Floor, walls and or ceilings soiled/has accumulation of debris. Throughout establishment, floors, walls and ceilings are soiled.
Floors not maintained smooth and durable. In kitchen, tile under sink and next to walk in cooler is chipped and showing concrete.
Food stored on floor. Several containers of margarine were placed on floor in kitchen and showroom area. Operator removed from floor.
Living/sleeping quarters that open directly into a public food service establishment without complete partitioning and/or self-closing doors. Dry storage area behind front counter with stored potatoes and margarine also contains a bed and chairs. Operator began removing food items.

Interior of oven/microwave has accumulation of black substance/grease/food debris. Microwave near live crabs at front counter area, top plate has encrusted residue.
