Warewashing machine not automatically dispensing the detergent and/or the sanitizer. Observed Dishwasher (Chlorine 0ppm). Operator changed sanitizer container, retest Dishwasher (Chlorine 100ppm).
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed at prep area. Operator cleaned the machine.
