Dead roaches on premises. Observed 6 dead roaches in back kitchen: 4 under utility sink by back door, and 2 between unused equipment near back door. Owner swept dead roaches during inspection.
Exterior door has a gap at the threshold that opens to the outside. Back door has gaps along bottom and side.
Floor soiled/has accumulation of debris. Floor soiled with old and fresh cooked rice and oil in back of kitchen under sinks and equipment.
Food stored on floor. Sauce stored on floor under woks, and several items stored on raised floor in walk-in cooler.
