Employee switched from working with raw food to ready-to-eat food without washing hands. Observed operator handler raw beef and then proceeded to work with margarine butter. Discussed with operator there must be a hand wash and gloves change in between working with raw and then ready to eat foods.
Employee washed hands with no soap. Observed food handler wash hands with gloves on with just water. Discussed with operator the proper technique for hand washing. Food handler removed gloves and washed hands with soap and warm water for at least 20 seconds.
Raw animal food stored over/not properly separated from ready-to-eat food. In the reach in cooler across from cook top, observed a container of raw chicken placed on top of cut ready to eat onion. Operator rearranged for proper storage.
Stop Sale issued due to food not being in a wholesome, sound condition. Employee switched from working with raw food to ready-to-eat food without washing hands. Handwash sink used for purposes other than handwashing. Observed hand washing sink located at the server station behind sushi bar being used as a prep sink. Discussed with operator hand wash sink are only for handwashing to prevent reduce cross contamination. Operator removed items from hand sink.
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Upon arrival no certified food manager present, total of 6 food handler employees working. CFM arrived at time of inspection.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed in use utensils in standing room temperature water by the rice warmer and next to fryer. Operator dumped water and ran utensils through dish machine.
A return visit. The problems from the previous inspection had been dealt with.

Paper towel dispenser at handwash sink not working/unable to dispense paper towels at the handwashing sink located near the three compartment sink. Operator was able to make the paperwork towel dispenser work at time of inspection.

In the stand alone cooler Raw beef was stored over raw shrimp. Operator rearranged all foods for proper storage.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. No time mark on fried Krab, per operator foods were cooked and prepared two hours before inspection started. Operator time marked foods.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Raw ready to eat fish removed from original packaging in the walk in freezer, operator stated it was removed from original packaging more than 7 days ago. No date mark on fish.
