Water leaking from pipe and/or faucet/handle. Pipe leaking under triple sink. Person in charge added catch bucket until it can be repaired.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Stock at 44F in three door lowboy at cookline. Person in charge states door shave been open frequently. Ambient temp of unit at 45F. Person in charge will monitor and move to other unit holding temperature if they do not drop.
Equipment in poor repair. Gaskets torn on three door lowboy at cookline. May be cause for temperature issues.
Floor soiled/has accumulation of debris. Observed grease buildup on floor along wall at cookline. Also, observed grease buildup on vent in back kitchen.

Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In reach in freezer raw chicken no longer commercially packaged stored over case of shrimp.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer bucket at 500+ppm, person in charge remade to 300ppm.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Operator unable to locate the test strips for quat sanitizer at triple sink and wiping cloth bucket.
Required employee training provided by a third-party employee training program and operator unable to provide original certificate for employees trained. Operator has photo copy of employee food safety training certificates. Operator ordered new certificates so that they were original but ordered incorrect certificates. New certificates only display Servsafe as food handler. Operator still has the photo copied certificates from previous inspection that still have a valid date. Operator stated she will order the correct employee food safety training certificates today.

Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloth stored on counter next to fryer on cook line. Operator removed wet wiping cloth to properly store.
