Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed salmon in ROP completely thawed inside walk in cooler. Operator discarded salmon onsite.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed sweet plantains (111F - Hot Holding) located at the steam table. As per operator, they were placed there about an hour ago. Operator began to reheat product.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed chlorine chemical test kit expired 12/01/23.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink next to three compartment sink. Operator restocked.
