Dumpster rusted out on bottom. Observed the bottom of dumpster in the front is rusted.

Food stored on floor. Observed water bottles stored on the floor in the walk in cooler, operator moved to a different area.
Hole in or other damage to wall. Observed 4x4 hole in the wall behind the shelf in the kitchen area, near bottom by some pipes.

Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours. Observed soups curry (129F - Reheating); lemon chicken rice soup (89F - Reheating) Reheating for 1hour 45 min in crock pot will not make temperature of 165 within 15 min at current location. Operator removed to quickly heat to 165F on stovetop.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Inspector provided handout at inspection via email.
