Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed mop sinks on 1st and 8th floor utilizing splitter with no vacuum breaker attached.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed 1st floor bar soda nozzle soiled.
Spray bottle containing toxic substance not labeled. Observed chemical spray bottle containing sanitizer stored near 1st floor 3 compartment sink with no label. Manager labeled bottle.

Observed an employee rub the nose then handle a fish filet. Operator disposed of the filet and had them do both operator wash their hands.
Handwash sink does not have enough water pressure to properly wash hands. Observed the upstairs hand sink with low water pressure.
Handwash sink used for purposes other than handwashing. Observed two hand sinks used as dump sinks. Operator cleared the sinks.

Observed in main kitchen area employee grabbed a raw ground beef patty with gloves on and placed it on the grill. The employee grabbed a burger bun with the same gloves. Person in charge stopped employee and had him discard bun,remove soiled gloves,wash hands and put on clean gloves.
