Chef stated the employee replenished cooler between 12:30-1:00, from the walk-in cooler. Chef placed all foods that were replenished back into walk-in cooler.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed back flow at mop sink incorrectly installed before splitter. Manager had employee remove splitter.
