Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Operator brushing hair out of face with bare hands then continued to handle clean equipment and clean bowl insert for rice cooker without washing hands between tasks.
Toxic substance/chemical improperly stored. Spray bottle of sanitizer, stainless steal cleaner, and all purpose cleaner on rack over clean rice cooker and utensils, bottles removed.
Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Missing advisory on menus. Also undercooked tuna on menu is not identified as being severed undercooked. Operator added consumer advisory at bottom of menu and added asterisk to tuna item on menu.
