To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. No proof of employee food handler training for Willard P., hired as server approximately 7 months ago and for Levar T., hired in February 2026.
Operator not able to provide monitoring, verification and corrective action records required to be maintained by the approved Process Waiver or HACCP plan. Establishment has approved cook-chill HACCP. Cooling temperature not recorded for any products using cook-chill on the six hours or less time column on June 22, 2026 records.

Operator stated all food was put into cooler at 10 am. All temperatures taken at 2:15 pm.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler under flat top grill on cook line, raw beef (60F - Cold Holding); raw salmon (60F - Cold Holding); raw shrimp (61F - Cold Holding). Operator stated all food was put into cooler at 10 am. All temperatures taken at 2:15 pm.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In steam table in cook line, sausage (120F - Hot Holding). Employee stated sausage has been in steam table for approximately 1 hour but he didn't have a cover on it to keep top sausage hot. Employee reheated sausage and rechecked at 187F.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. On cook line, Sanitizer Bucket (Quaternary 500+ppm). Operator had employee discarded sanitizer bucket and made new. Rechecked at 150ppm.
Non-pitting surface rust on food-contact equipment. Surface rust on counters of metal deflector plate in both ice machines.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach in cooler on cook line under flat top grill. Thermometer on machine reading 60+ and ambient temperature of cooler is 66F. Operator stated she will have someone out to look at soon as possible.
Employee chewing gum while preparing food or while in a food prep/food storage/warewashing area. Employee in alley chewing gum while bagging up to go soups for customer. Operator had employee discard gum in trash.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee bag on case of breadstick bags in cabinet in side room with dessert cooler. Operator removed employee belongings and placed in office.

Metal pans stacked before of cheese blocks filled to very top. Pieces of cheese on bottom of pan at in temperature, top cheese not in temperature. Operator removed top cheese and placed back in walk-in cooler to bring down temperature.
Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. In back prep area, Sanitizer Bucket (Quaternary 500+ppm). Operator had employee remake sanitizer bucket and rechecked at 200ppm.

Server in dish area handled soiled plate, placed plate in dish area, then handled clean cheese grater without washing hands and placed it with other clean cheese graters on pass through line. Manager coached employee on proper hand washing procedures. Manager also removed all cheese graters to be washed.
