Log for today is not filled out, log from yesterday is missing, as well as missing logs through out the last 6 months. ( logs are not kept in chronological order).
Unable to access temperatures from continuous monitoring devise for the last 6 months. Manager was able to access the incidents ( alarm alerts for last 6 months).

In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handle stored in flour at super Breader bin. Manager removed scoop and sent to ware washing area.
Stored food not covered. Pasta, beef block and sausage block stored in walk in freezer not covered.
Wet mop not stored in a manner to allow the mop to dry. Manager removed mop from bucket and hung to dry.
