Also observed precooked chicken wings 47f, beer cheese 46f (unknown amount of time that items have been in cooler per manager). Manager discarded all items during inspection.

Upon inspection, observed shredded cheese 46f in lower make bar cooler # 2 (unknown amount of time per manager). Accumulation of dead flying insects in control devices. Upon inspection, observed approximately 14 dead flying insects on sticky trap in kitchen area of establishment. Manager discarded sticky trap during inspection.
No handwashing sign provided at a hand sink used by food employees. Upon inspection, observed no handwashing sign located near hand wash sink in center bar area in pool table area. Manager was provided a handwashing sign during inspection.
Unwashed fruits/vegetables stored with ready-to-eat food. Upon inspection, observed unwashed onions stored on lid of container containing ready to eat lettuce in walk-in cooler.

Observed 10 dead roaches in the following locations: 1 behind loose board behind icee machine in snack bar on bowling side, 1 on low shelf in snack bar, 1 under grill in kitchen, 1 under shelf in dry storage, 1 by closet behind main bar, 3 in closet behind main bar, and 1 in cabinet next to handwashing sink at main bar. Operator cleaned and sanitized areas during inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed mold like substance on wood under hand washing sink in sink under hand washing sink at drink bar on bowling side.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. Observed hole on side of building with visible insulation.
Stored food not covered. Stored shrimp, egg rolls, and pretzel bites did not have lids at time of inspection. Operators placed lids on items during inspection.
Waste receptacle/floor mat/hood filter cleaned in a manner that allows the waste cleaning water to drain onto ground. Waste water from bar disposed of on ground as evident of clean out hose, drink straws, lemon wedges, and bottle caps on ground around outside drain.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Upon inspection, observed 0 ppm reading for dishmachine at establishment. Manager contacted EcoLab during inspection and technician was able to repair dishmachine during inspection. 50 ppm reading was observed.
