Manager stated door was opened and closed quite a bit as cheese was close to the door. Manager had employee place cheese in reach-in freezer to reduce temperature to 41F.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Hose side of splitter in mop sink.
Food-contact surface soiled with food debris, mold-like substance or slime. Slicer on prep table next to back door soiled with old food debris.
