Handwash sink not accessible for employee use at all times. In the server's area the hand washing sink is blocked by a cart filled with plates and bread baskets. Operator moved the cart from in front of the hand sink.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Provided operator with copies of the employee health reporting agreement, employees began reading and signing the agreements.
Equipment in poor repair. The tops to the naked table cooler on the cook line are damaged.
