Employee dried hands on clothes/apron/soiled towel after washing. Employee wiped his glove hands on his apron. I informed the manager and he had the employee to change gloves wash his hands and put on a new pair of gloves.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Chicken 48F. Cook line.
Microwave missing fan guard cover/splatter shield. Microwave above the expo line and fry warmer. 1st microwave.
Employee with no hair restraint while engaging in food preparation. Employees engaging in food prep on the cook line. The manager provided the employees with hairnets.

In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Handle inside the mixed cheese at the salad station is inside the cheese. The handle was removed from out of the cheese.
Interior of microwave has accumulation of black substance/grease/food debris. 1st microwave on the fry station.
Wiping cloth sanitizing solution stored on the floor. The manager doubled the bucket to get the bucket from off the floor.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee used his cell phone with gloves on and proceeded to cut cilantro. The manager the employee to remove the gloves and wash his hands.
No paper towels or mechanical hand drying device provided at handwash sink. Hand washing sink near the 3 compartment sink. Paper towels was replaced.
Clean equipment and utensils not separated from soiled equipment and utensils at warewashing area. Soiled pan stored with clean pans on the shelf across from the 3 compartment sink. The soiled pan was removed to be washed, rinsed and sanitized.
Equipment in poor repair. Handle missing on the 3rd microwave across from the grill.

Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Thermometer tested at 143f.
