Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee removed trash can, squeegeed floor, took boxes out side, came in side on cook line, took off gloves, replaced gloves no hand wash.
Ceiling/ceiling tile shows damage or is in disrepair. Hole in ceiling tile on exposed line. Ceiling tiles moved leaving open gaps above ice machine.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Vents soiled in ware washing area.
