Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee entered kitchen from rear exit door and began handling clean equipment and clean utensils used to prepare, serve and store food without washing hands. Operator discussed with employee who then washed hands.
Employee switched from working with raw food to ready-to-eat food without washing hands. Employee handled raw hamburger and then raw chicken while placing to cook then handled buns while placing in toaster without washing hands. Operator discussed with employee who then washed hands.
Live, small flying insects found. Approximately 20 live small flying insects in dining room flying around soda dispensing equipment. Advised Operator to eliminate live, small flying insects, clean and sanitize area.
Rodent activity present as evidenced by rodent droppings found. Approximately 3 rodent droppings in kitchen on floor under service counter. Advised Operator to remove rodent droppings, clean and sanitize area.
Dead roaches on premises. 5 dead roaches in kitchen on floor under/behind equipment/shelving. Advised Operator to remove dead roaches, clean and sanitize areas.

Employee handled customer payment/point of sale system/headset attached to face/ear and then handled clean utensils to scoop hash browns and ice to prepare food/drinks for customers without washing hands. Operator discussed with employee who then washed hands.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. At reach in freezer; opened bag of raw chicken stored on top of opened bag of ready to eat potato fries. Operator moved raw chicken away from fries.
