Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted and approved within 60 days. Observed three compartment sink was removed also dishwashing machine and hand wash sink was added without plan review approval.

Handwash sink used for purposes other than handwashing. Observed hand washing sink used to washed coffee thermos and bread trays.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed to chlorine test strips available at time of inspection.
