Food-contact surfaces: The fans in the walk-in cooler are soiled with dust and a mold-like substance, and the can opener is soiled with food debris. The operator cleaned the can opener.
Dead roaches on premises. 6 dead roaches were found under the prep table on the floor, on the left side of the three-compartment sink. 3 dead roaches were found behind the reach-in single freezer across from the dishwashing area, on the left side. The operator swept up the dead roaches and sanitized the area.
Food storage container/container lid cracked or broken. 2 cracked containers on the storage rack. The operator discarded the two cracked containers.
