Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink, splitter added, missing backflow preventer on non chemical dispenser side with silver metal hose attached.
Equipment and utensils not properly air-dried - wet nesting. At dish area, metal and plastic containers stacked before air drying.

Employee placed soiled rice container down, vent cover on back of reach in cooler fell off onto floor, employee picked up cover and placed cover on and immediately began preparing food for guests without washing hands. Manager coached employee on proper hand washing procedures.

Employee on cook line handling raw chicken bags, placed contents on grill, placed bags in trash container and pushed down with gloved hands and immediately went back to preparing chicken on grill without washing hands. At front counter display area, cooked steak 124F, cooked chicken 118F and cooked chicken 120F. Items cooked approximately 15 minutes prior. In warmer by cook line, five pans of rice 110F. Rice placed in warmer approximately 2.75 hours prior. All items voluntarily discarded.

Toilet/urinal not flushing/functioning properly. Toilet in women's restroom in dining area. Manager states repair company has been contacted.
