Vacuum breaker missing/incorrect position at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuumed breaker installed at top of splitter added at mop sink. Vacuum breaker missing from non-chemical side of splitter with attached hose.
Floor soiled/has accumulation of debris. Under cooking equipment along back wall has debris build up.
